Referral Information

Hospice of Waterloo Region accepts medical and non-medical referrals for individuals who have been given a palliative diagnosis, are in their last year of life and require emotional or practical volunteer support or for caregivers requiring respite care or bereavement support.

A Hospice of Waterloo Region staff member will conduct an in-home assessment to determine eligibility for Hospice services and to recommend the Hospice service(s) that would be most appropriate and helpful.
