Getting Started

Below is the process for becoming a Hospice volunteer.

1. Review Personal Inventory for Prospective Hospice Volunteers
2. Download an application form by clicking here
3. Complete application form. Include the names of three references.
4. Mail completed form to Hospice of Waterloo Region, 298 Lawrence Avenue, Kitchener, Ontario N2M 1Y4
5. Attend screening interview with Hospice representatives.
6. Attend orientation and training program and submit screening documentation (police check, driver’s abstract (if applicable), immunization and insurance verification).
7. Attend graduation interview to discuss placement opportunities.
