As a team of Advance Care Planning (ACP) Ambassadors here at Hospice of Waterloo Region, we are ALWAYS looking for new ways to help the general public understand what these ACP Conversations are all about. Our goal is to get keep the information coming in order to build your confidence in having your own ACP conversations.

In the past year, we have heard from lawyers, estate planners, leaders in long term care, hospice care and community paramedicine. Most recently we hosted an incredible panel of intensive-care unit staff who definitely made us sit up and listen. 

We have heard your stories and shared some of our own. Every conversation brings new learning and we are not finished yet. 

On March 31st, we invite you to join us as we welcome a local treasure and an expert in caring for the dying, recently retired Palliative Care Physician, Dr. Donna Ward.  

ABOUT THE EVENT

This is a virtual event. Using a Q&A format, Donna will share her own observations about choosing/being someone’s SDM, as well as what to consider in preparing the SDM to make decisions on your behalf.     

With time for facilitated discussion, our guest and our expert team of ACP Ambassadors will explore the answers to the questions you might not even know you need to ask!
